2/26/2024 EnchantaVaughn Approved LMHPR held an individual session with Ms. Broadnax. The purpose of today’s session was to focus on Ms. Broadnax mental health. Upon arrival at Ms. Broadnax home, Ms. Broadnax appeared withdrawn, evident by her facial expression and body language. Upon arrival, she did not present any symptoms of S/I. LMHPR began session by exploring with Ms. Broadnax her current emotional state. LMHPR reviewed with Ms. Broadnax triggers to her depressive states and her interactions with her friends and family members. LMHPR explored positive coping skills and reviewed Ms. Broadnax medication management. LMHPR explored with Ms. Broadnax ways to improve her mental state and encouraged Ms. Broadnax to utilize conflict resolution along with problem solving skills to address her current situation. LMHPR engaged Ms. Broadnax throughout the session through the use of active listening, asking open-ended questions and providing empathetic responses. LMHPR ended session by reviewing interventions with Ms. Broadnax and encouraging her to provide feedback on the session. Ms. Broadnax actively participated in the session. While exploring with Ms. Broadnax her current emotional state, Ms. Broadnax shared that she was upset. Ms. Broadnax stated during the session that she did not want to be friends with one of her long-term friends anymore, so she blocked her from calling her. While exploring problem solving and conflict resolution skills, Ms. Broadnax stated that she blocked her friend’s number because she did not want to engage in a verbal altercation with her over the phone since she was very upset. Ms. Broadnax stated that she was concerned about becoming physically aggressive, so decided to refrain from any interaction with her friend at this time. At the end of the session, Ms. Broadnax shared that she felt people were unreliable and will be mindful of her trust towards others. LMHPR will continue to offer therapeutic techniques to assist Ms. Broadnax in processing symptoms and roots of depression. LMHPR will make plans to meet with Ms. Broadnax on 2/27/2024.
